Please note: 

There will be a charge for letters for Third Party organisation to confirm diagnosis of conditions from your medical records. 

For DWP, Blue Badge etc your own evidence can be provided from letter sent to you directly.  Any additional information required from the GP will be requested directly from the DWP or Council to the surgery with patient consent. Response to these requests are up to 30 days. 

 APPOINTMENTS WITH A HEALTHCARE PROFESSIONAL WHEN YOUR GP PRACTICE IS CLOSED 

MONDAY TO FRIDAY 6.30 TO 8PM 

SATURDAY 9 AM TO 5PM

This service is not available for repeat prescriptions, medical certificates our routine requests.
